As a full service Ophthalmology practice, San Luis Obispo Eye Associates’ physicians are leaders in their fields and use state-of-the-art equipment and techniques to provide you and your family with the best possible care. We look forward to serving you and helping you take care of your eyes.

No Show/Cancellation Policy

As a courtesy to the practice, please call the office to reschedule your appointment as soon as you can. We appreciate 24 hours notice so that we may offer that time to another patient.

New Patient Forms​

Please fill out the first 3 forms before your appointment:

Prescription Refills

Please contact your pharmacy when you require a refill of your Ophthalmologic (eye) medications. The pharmacy will contact our office even if you have no refills remaining. You may also choose to request prescription refills in our Patient Portal.

New prescriptions and refills are sent to most pharmacies electronically and will be processed on the same day as they

are approved/requested (unless the request is received after hours and then it will be taken care of the next business day).

 

Please try to request prescription refills at the time of your visit. Requests for emergency prescription refills initiated by either you or your pharmacy to our on-call physicians after hours or on holidays or weekends will incur a fee of $10 per prescription.

After-Hour Emergencies

Regular Office Hours are Monday thru Friday from 8 am - 5 pm. Anytime you have a life threatening emergency - Please call 911. We do provide emergency coverage for our patients 24 hours a day, seven days a week.

If you have an eye related emergency after hours, call either of our offices (805) 781-3937 or (805) 434-5970 to speak with our answering service. They will contact the Physician on-call for after hour emergencies for our practice. Requests for emergency prescription refills initiated by either you or your pharmacy to our on-call physicians after hours or on holidays or weekends will incur a fee of $10 per prescription.

If you need to speak with someone in our office and it is not an emergency, feel free to leave a message with our general voicemail box and your call will be returned the same or next business day.
