Appointment Information

For Your first visit:

Please arrive 15 minutes prior to your appointment to allow time so we may scan your insurance card(s) and completion of any paperwork unless instructed otherwise.
It is very important you bring the following items with you to your appointment:

  • Current Insurance Card(s)
  • Current Glasses
  • List of current medications and eye drops, including vitamins and herbal supplements
  • Sunglasses (to protect your eyes when leaving in case of dilation)

It is common practice to have your eyes dilated on your first visit with an Ophthalmologist. Eye drops are used to dilate your pupils for the doctor to see into your eyes to look at your retina. Dilation may last up to 3-4 hours and may cause your eyes to be light sensitive and blurry. Therefore, patients may wish to arrange for transportation home from their initial appointment.

Children under the age of 18 must be accompanied by their parent/guardian. For children between the ages of 14-18, the patient can bring a letter signed/dated by their parent/guardian stating it is okay for San Luis Obispo Eye Associates to treat the minor patient. Note should include patient’s full name, date of birth, date of appointment and physicians name the child will be seeing.
